Medical/Physical Information

PHYSICALS/HEALTH HISTORY UPDATE: 

All students who participate in the athletic program are required to obtain a physical examination as per NJ state law.  A new physical is required every 365 days.   Students can obtain the physical examination from their private physician or from our affiliated doctors.  

All forms are to be submitted to the school nurse.

 Mandatory Forms (must have one completed each season):  

Spring Pre Participation Physical Exam Form OR Spring Health History Update

SPORT REGISTRATION: 

To be eligible for sports ALL INTERESTED ATHLETES must register.  

General Information:  Athletic Registration Website, even if they completed registration for another sport.
