New for Spring 2026 Sports - Medical Eligibility Forms and Health History Update Forms will now be uploaded to Arbiter Sports as part of the registration process.

Welcome! To help your child get started in our athletics program, please take a moment to carefully read and follow the registration steps outlined below. These instructions will guide you through enrolling your child, ensuring they’re set up for a fun, active, and successful year. Let’s get started!

Step 1

Print the Pre-participation Physical Evaluation Medical Eligibility Form to take to your child's pediatrician at the time of their scheduled annual physical examination. Your child's pediatrician is required to be licensed in the state of New Jersey.

Once the form is completed, you are required to upload it to your child’s online athletic registration account in Arbiter Sports for medical clearance to participate in all fall, winter, and spring sports.

Please note that Medical Eligibility forms are valid for 365 days from the date of the physical examination.

Step 2

Print and complete the Health History Update Questionnaire, then upload it to your Arbiter Sports account. This form is only required if your child’s physical exam was done more than 90 days before the first official practice.

High school questions may be addressed during school hours by contacting the MOHS health office at 973-927-2208.

Middle school questions may be addressed by emailing the MOMS health office at moms-nurse@motsd.org

Resources for Sports Physical Examination

Those with health insurance must utilize their health benefits to fund the physical.
The Skylands Orthopedic offices will be free of charge through the district to those who do not have access to medical insurance. Please get in touch with Skylands directly (908-813-9700) to schedule your child’s physical. It is highly recommended that families call as soon as possible to schedule an appointment to ensure there is sufficient time to secure a physical.
