PHYSICALS/HEALTH HISTORY UPDATE: 

All students who participate in the athletic program are required to obtain a physical examination as per NJ state law.  A new physical is required every 365 days.   Students can obtain the physical examination from their private physician or from our affiliated doctors.  

All forms are to be submitted to the school nurse.

 Mandatory Forms (must have one completed each season):  

Pre Participation Full Physical Exam Form OR Health History Update 90 Day

SPORT REGISTRATION: 

To be eligible for sports ALL INTERESTED ATHLETES must register.

Important Fall Sports Registration Update

Attention athletes and families: Please consult with the school nurses to determine whether your child requires a new physical examination or an updated health history form for the upcoming fall sports season.

This process has changed due to rSchool’s transition to ArbiterSports, which requires all families to create a new account in order to register their athlete(s).

Sports Physical Requirements

To be eligible for participation, physicals must be valid on or after the following dates:

• Football & Girls’ Tennis – August 11, 2025

• All Other Fall Sports – August 18, 2025

Please note: Even if your athlete has been approved for summer practices, a new physical must be submitted if their current physical will expire prior to the dates listed above. All physicals must be reviewed and cleared by the school nurses and team physician before athletes may participate in official fall practices.
