Updated Sports Physicals MEMO

Dear Mount Olive Families,

Registration will soon open for Winter Sports. Student athletes must have a physical exam completed each year for participation in school athletics and submitted to the school nurse for clearance. 

Students are encouraged to seek physicals from their trusted healthcare providers. Consistent care is ensured by involving a student's primary healthcare provider, allowing for ongoing medical attention and personalized care based on their medical history. 

While physicals may also be obtained from our district doctors, these physicals will now take place at Skylands Orthopaedics in Hackettstown instead of our school grounds. This location change is necessary to provide the most appropriate environment for medical exams and to accommodate the growing number of students who are seeking physicals through the school doctors. The Skylands Orthopedic offices will be free of charge through the district to those who do not have access to medical insurance. Those with health insurance will be required to utilize their health benefits to fund the physical. Please reach out to Skylands directly (908-813-9700) to schedule your child’s physical. It is highly recommended that families call as soon as possible to schedule an appointment to ensure there is sufficient time to secure a physical. 

Please note:  

  • Once the physician completes your physical, please bring it to your school nurse.  The school nurse will review the paperwork and send it to our MOTSD school doctor, who will provide final clearance. 

  • Make doctor appointments early to meet deadlines for athletic clearances. Physical forms are due to the school nurse by Monday, October 23rd.  

  • Physical forms may be completed from other facilities if there is a wait to see your doctor.

Additional Resources to Secure Sports Physicals (Please note the district does not have an affiliation with these groups. Families will be responsible for the fees associated with securing a physical from these groups regardless of insurance status.)

PHYSICALS/HEALTH HISTORY UPDATE: 

All students who participate in the athletic program are required to obtain a physical examination as per NJ state law.  A new physical is required every 365 days.   Students can obtain the physical examination from their private physician or from our affiliated doctors.  

All forms are to be submitted to the school nurse.


ADDITIONAL MEDICAL FORMS:

If you have any of the following conditions (asthma, required to carry an Epi Pen, diabetes diagnosis, or susceptible to seizures), please complete and hand in the appropriate forms: (If not already submitted to the Health Office for the school year).
